Before we can begin an evaluation or any tutoring sessions, we need to gather some information from you:  a student information form, a parental consent form, a release of information form, and our business policy/financial agreement. The forms are attached below in one enrollment package for your convenience.   You may mail these to us or bring them to our first meeting.
